Accident and health individual and blanket form filings, 1960-1981.

Accident and health insurance policies required to be filed by insurers under section 631.20, Wis. Stats. Master policies and certificates are included for individual and blanket insurance. Certifications of readability, filed by insurers and stating that all personal consumer form filings meet minimum standards for reading ease, may be included. Initial certifications were required to be filed with new forms beginning June 1, 1981, under section 6.07, Wis. Adm. Code.
